Insurance, Self-Pay & Refund Policy

Insurance Verification

Our office verifies your insurance coverage one business day before your appointment. If you provide valid insurance information, we will bill your visit to your plan. You are responsible only for your copay, deductible, or coinsurance as determined by your insurance provider. You do not need to request prior authorization yourself unless your plan specifically requires it.

Self-Pay Visits

If no insurance information is provided before your appointment, you will be considered self-pay.

  • Initial evaluation: $275
  • Follow-up visit: $ 99

Refunds & Credits

If you make a payment before your insurance claim is processed, that amount will be applied to your copay, deductible, or coinsurance.

  • If your payment exceeds your responsibility, the balance will be applied as a credit to your account or refunded upon request.
  • Self-pay payments are applied directly to visit charges.

We are committed to clear, fair billing and will ensure you are never charged more than your actual financial responsibility.
